In a multicenter vignette study, internal medicine residents first decided alone and then deliberated in small peer groups. Discussion reduced practice intensity and increased clinical certainty beyond the result expected from averaging individual scores. Practice intensity meant willingness to order tests, consultations, or treatment. That result offers narrow evidence that discussion can lower action intensity through interaction. Still, the correspondence does not establish better decisions. The instrument did not measure whether less intense practice was appropriate, and the study did not examine autonomous agents or work-product approval.
